•No working spots or auditing spots will be held without full payment.
•Auditing/Working spots are not exchangeable - each person must register.
•All working spots are on a first come, first fill basis until all spots have been filled - a waiting list will be created
after that.
•If a person cancels no refund will be given, (exceptions MAYBE made for working spots IF notification is given 48
hours before the workshop AND a person from the waiting list is able to fill that spot and their payment IS
received 24 hours before the start of the seminar).
•All payments for the seminar, (auditing and working) will be deposited immediately upon receipt. Payments from
any persons having to go on the ‘waiting list for working spots’ will be refunded unless otherwise instructed to
apply a portion of the payment towards an auditing spot whereby a refund will be given for the remaining
amount.
•No Choke Chains, Prong Collars, Flexi-leads or similar will be allowed on working dogs (please use buckle collars
or front-clip harnesses with a 4' or 6' leash)

Seminar/Workshop Release Form
Please read over, sign, date and return with Registration Form
- - All persons attending, (working and auditing), must sign this release form - -
I, ________________________________________, understand that on February 26, 2012 I will be participating
in a Paws-a-tively Canine! Seminar/Workshop at 210 1/2 1st Ave., Laurel. I agree to abide by the rules and
regulations of Paws-a-tively Canine for their building and any other rules posted or stated by Paws-a-tively
Canine!.
I understand that if I hold a working spot I am only allowed to bring the dog I have registered with to the
seminar/workshop and this must be the same dog for the whole day. I state that my dog is in good physical health
and current on all needed vaccines, wormed, is 6 months of age or older and is tolerant of other dogs and
people. I agree to keep my dog in a secure crate or on a leash at all times while at the seminar/workshop and on
a leash at all times when on the grounds and to carry potty/clean-up bags with me before, during and after the
seminar/workshop to clean up after my dog.
I understand that working with a dog is a physical sport and requires moving and bending for both myself and my
dog. I state that I am in good physical health and am able to participate in this workshop. I take full responsibility
for my actions and my dog’s actions at this event before, during and after this workshop. I acknowledge all
hazards presented by this event and the event premises to my self and my dog.
I agree to hold harmless Paws-a-tively Canine!, their staff, volunteers, and/or board of directors for any and all
injuries, loss, and damages to persons or property arising from, in connection with, or during function or activity
while at this seminar/workshop and to hold the aforementioned parties harmless from any claim for loss or injury
which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog while
in or about the event premises or grounds or near any entrance and I personally assume all responsibility and
liability for any such claim; and I further agree to hold the aforementioned parties harmless from any claim for
loss, injury or damage to this dog.
Kritterkare’s Paws-a-tively Canine! shall not be responsible in any way for personal equipment or belongings that
may be damaged or lost by fire, theft, accident, or otherwise. I understand that there will be no refunds once
payment is received and deposited.
